Building Sewer Information Request Form

By submitting this form, I hereby request to obtain a copy of the building sewer information/location at the above referenced property.  It is understood that the Hamilton Township Municipal Authority may not have a detailed drawing for all customers, from when the initial building sewer line was installed.  However, if this information is available for this request, the Authority does NOT guarantee the information is not without error.  The recipient of this information acknowledges the above, and by signing this Request and Release of Liability, agrees to hold the Hamilton Township Municipal Authority harmless and free from any and all liability as a result of the information provided.  It is the recipient's responsibility to validate and locate the building sewer prior to the start of any and all work on the owner's property.
